I took a group of lower level people to the cursed woods not so long ago. Flamey adjective things attack us and all of them must have typed 'kill fla' - it was almost a bloodbath as I started hacking through them. Fortunately I realised their mistake and fled. It turned out they were all just letting the attack continue, wondering what was going on!

I shoulda killed them all for the insolent behaviour! (At least I could put 3 kills to my name)

You sure about that? I, personally, type "kill fla" everytime you walk into a room. Most times, I don't hit enter.

Most times.

Seriously, though, there should also be a warning of sorts for things like this. For instance, attacking a fellow group member, leader, etc.(don't make this impossible, but perhaps have them enter the command twice?)

I know I've done it and not even with an abbreviation. The full name of the person. I start typing out what I want to do, c 'acid blast' then someone speaks and I am last see first thing typed sort of person, and it becomes that person rather than some dwarf.
